Symptoms
Plantar fasciitis typically causes pain on the underside of the heel or along the arch of the foot.
Common symptoms include:
- Sharp pain under the heel
- Pain around the instep or arch of the foot
- Pain that’s worse first thing in the morning
- Discomfort after long periods of sitting
- Pain that eases as you walk before returning later in the day
- Tenderness when pressing on the heel
- Pain after prolonged standing or walking
- Reduced walking or running tolerance
In the early stages, symptoms often improve once you’ve been moving for a few minutes. As the condition progresses, the pain may become more persistent and begin to affect everyday activities.
Causes
The plantar fascia is a strong band of connective tissue that runs from your heel bone to your toes. It acts like a spring, supporting your arch and helping absorb forces every time you walk or run.
Rather than being purely an inflammatory condition, we now know that plantar fasciitis is usually the result of repetitive overload, causing small areas of tissue degeneration within the plantar fascia. The body’s ability to repair the tissue can’t keep up with the demands being placed upon it, leading to pain and reduced function.
Common contributing factors include:
- Sudden increases in walking or running
- Changes in training volume
- Standing for long periods
- Flat feet or excessive foot pronation
- Tight calf muscles
- Weakness in the foot or lower limb
- Changes in footwear
- Increased body weight
Several factors often combine to overload the plantar fascia, which is why a thorough assessment is essential.
Diagnosis
A detailed physiotherapy assessment is usually all that’s required to diagnose plantar fasciitis.

Because the plantar fascia has a relatively limited blood supply, recovery can take time.
Typical recovery expectations include:
- Mild cases: 6–8 weeks
- Moderate symptoms: 2–4 months
- Long-standing cases: Several months with progressive rehabilitation
Most people notice gradual improvements as strength, tissue capacity and movement patterns improve.
